have you checked the decompression valve? It’s good practice to lap it with some lapping paste on the cone.
Have you checked the valve inside the carter? (Where the crankshaft rotates). If that’s ruined, you will have all sort of problems at low rpm, even with bigger main jets.

this collector is made specifically for Piaggio Si and Piaggio Bravo.
On the Ciao you’ll need to cut part of the frame in order to fit it.

i’ll try to explain in english, i’m from italy.
You could try to cut a piece from a can of beer or cocacola and insert it in the exhaust collectior.
Tin is a very soft metal and will squish as you tight the bolt. You could add a bit of paste if needed.
I did this on my original piaggio si and it worked flawlessy.

I was looking for a Malossi MP One kickstarted side crankcase. I have a Piaggio Si kickstarter and i want to keep the reed valve system on it.
I took the screenshot from web.archive.org since their website is offline
I think it’s a particular part and not many people bought it, so i don’t think i can find it used.
